What is the FCCP?
The Family Care Community Partnership (FCCP) is a prevention program funded by the Rhode Island Department of Children, Youth, and Families to support the success of families living in the state of Rhode Island. The FCCP brings community–based service providers, family members, and friends together to build a stronger, brighter future for your family. Whether experiencing issues within the school, home, or community, the FCCP can help your family develop a plan, mobilize natural/formal supports, and assist you as you work toward achieving your goals. The FCCP assists families in navigating services or identifying resources that may be available to help their family.
The FCCP provides Wraparound services to support families in the following areas:
- Educational Concerns
- Housing Insecurity
- Financial and Employment Concerns/Needs
- Parental and/or Child Substance Use
- Parental and/or Child Mental Health, Behavioral, and Developmental Needs
- Domestic and Community Violence
- Legal Status / Immigration Resources
- Youth Diversion Program (YDP) and Wayward Disobedient Petition (WDP)
- Other factors causing parental and family stress or creating barriers to meeting basic needs such as food, clothing, transportation.

Description
The Family Care Community Partnership (FCCP) brings community-based service providers, family members, and friends together to build a stronger, brighter future for your family. The FCCP is a prevention program funded by the Rhode Island Department of Children, Youth, and Families (DCYF) to support the success of families living in the state of Rhode Island. Participating in the FCCP program does not mean that your family is involved with DCYF.
Eligibility
A family may be eligible for the FCCP program if they meet the following criteria:
- The family has one or more children 0-17 years of age who are presently living within the home, within the care of their parent(s) or guardian(s).
- The family is able to meet with an FCCP worker in person weekly for 30-60 minutes.
And have one or more of the following areas of need:
- The family is at risk of DCYF involvement due to potential for abuse/neglect
- Risk factors such as behavioral health, mental health, substance use, and/or domestic violence
- Involvement with Juvenile Justice (probation or leaving RI Training School)
- Youth Diversion Program (YDP) 9-17 or Wayward Disobedient Petition (WDP) 12-17
- In need of referral to services that support with basic needs (food, clothing, transportation, housing/shelter), employment, education, mental health/medical/dental needs etc.
The family does not need to be involved with DCYF to participate in the FCCP program.
Referral
Anyone can make a referral to the FCCP, including families, community providers, etc.

Support for Military
Child & Family’s FCCP program provides support to families who live in the East Bay area of Rhode Island including Active Duty, Reserve, National Guard, and Veteran families. This program is free and available to anyone who qualifies. Contact us today or look for more information about us at Yellow Ribbon events.
Location
Child & Family’s FCCP serves the east bay region of Rhode Island, working with families residing in the following cities/towns:
- Barrington
- Bristol
- Warren
- Tiverton
- Little Compton
- Portsmouth
- Middletown
- Newport
- Jamestown
The FCCP is knowledgeable in local and state resources that may assist your family in meeting their goals.
What to Expect
Following referral your family will receive a phone call from the FCCP to provide you with an overview of services, gather information regarding your family’s needs, and schedule an initial appointment with your family.
During your first appointment, a FCCP worker will review and complete intake documents and continue to gather information regarding needs.
Following this, your assigned FCCP worker will meet with your family in-person weekly and will provide an additional check in phone call once per week. Meeting weekly is key to the FCCP being able to assist your family in meeting their goals, due to this expectation, your worker can meet with you in your home or community.
Your assigned FCCP worker will complete assessments to gain a better understanding of your family’s needs, strengths, and supports.
Your assigned FCCP worker will assist you in developing a “family vision” that outlines your hopes and goals for your family’s future. Using this vision, and the information gathered from assessments, your FCCP worker will assist your family in developing a comprehensive and individualized plan with outlined steps to meeting your identified goals.
Wraparound Team
A core component of the Family Care Community Partnership (FCCP) is assisting families in creating a Wraparound Team or Village of supports to assist them through the wraparound process and beyond. Your Wraparound Team will include family members, friends and community members that are committed and supportive of you and your children! FCCP will assist you in facilitating meeting(s), at least monthly, with your Wraparound Team in which the team develops a team mission geared towards your goals, celebrate your successes and address barriers as they arise.
Typical Wraparound Team Members:
- Family: Parents/caregiver(s), grandparents, aunts/uncles, or extended family members.
- Friends: friends, neighbors, significant others, or youth/peers.
- Community: neighbors, teachers, pastors/priest, social service providers, etc.
- Anyone else that you would like to include!
